To provide optimal care for each individual patient, we have chosen not to join insurance networks. This allows us to follow ADA guidelines in planning your dental care and utilize our best resources.
We strive to provide you with affordable, high-quality care that fits within your budget. We offer various payment options to meet your needs. Our office accepts payments by cash, checks, and all major credit card. If you have any questions about payment, please ask us!
Financial Agreement
- I understand that the office does not participate with any dental insurances directly.
- I understand that the office will assist with submitting dental claims. The insurance company will then reimburse myself directly.
- I understand that all payment is due at time of service.
- I understand that if I begin major treatment that involves lab work, I will be responsible for the fee at that time
- Treatment plans may change, and I will be responsible for the work completed.
- If sent to collections, I agree to pay all related fees and court costs.
- I agree to pay finance charges of 1.5% per month (18% APR) on any balance 90 days past due.
- I will pay a fee ($75) for appointments broken without 48 hours' notice.
- If paying with a check, a fee ($50) will be applied if check is returned.
